Expectations
Patients are asked to arrive promptly for their scheduled appointments.
New patients and patients with new or updated insurance information
are asked to arrive 30 minutes prior to their schedule appointment to complete
paperwork.
During the examination, please have a list of all medications needing refills. This will limit
delays with refilling your prescriptions. Remember, antibiotics or pain medications cannot be refilled
without seeing a provider.
Appointments
All visits are by appointment only. Walk-in appointments are not available.
Please call (770) 941-4810 between 8:30 a.m. and 4:00 p.m. to schedule an appointment.
Scheduling may be disrupted, so we can accommodate emergencies. We thank you for
your understanding and patience.
Test Results
All normal test results will be reported by a postcard. All abnormal results will be reported by phone call. Please allow one (1) week for standard lab work to be received and reviewed by your provider. Pap smears may take up to four (4) weeks to receive the results.
Referrals
Referral requests are reviewed and/or generated within 72 hours. If you make an appointment with a specialist and your insurance requires a referral, you must notify us as soon as possible, so we may obtain a referral authorization for you. Please do not expect to get same day referrals, this is usually impossible.
Medical Forms
To expedite your visit, you may download and complete the following form(s) prior to your
